Aging Life Care:

Aging Life Care Professionals work with older adults and other individuals facing ongoing health challenges. Their guide families towards the actions and decisions that ensure quality care and an optimal life for those they love, thus reducing worry, stress and time off from work for family caregivers through: assessment and monitoring; planning and problem-solving; education and advocacy; family caregiver coaching. Care managers may work independently, through certain care agencies or with skilled nursing and rehabilitation centers and providers. Check below for some local life care managers.

Aging in Place Services:

Aging in Place (aka Living in Place) is the concept of remaining in your own home and community – safely and independently as you get older. Aging in Place Services are grassroots organizations, run by area residents, that help older adults remain in their homes. The types of services they may provide include: cultural and social activities; referrals to health care providers and tradespeople; transportation to shopping (please also refer to Transportation Services (on page 20), personal care and medical appointments; health and wellness programs. Adult Day Programs:

Many of the adult day programs are currently on hiatus due to Covid but expect them to reopen when it is deemed safe for attendees and staff. Typically, these programs serve people who might be isolated in their homes and also provide respite for caregivers; these programs provide meals and engaging social activities. AARP Foundation Tax-Aide Program 
Free assistance for low to middle-income taxpayers in getting their tax returns processed. Program usually starts early February in 20 libraries and other sites throughout Westchester county;

Family Caregiver Support Program, Livable Communities Livable Communities are informal services to help seniors remain in their own homes. Safe Centers for Seniors
Westchester County’s support for at-risk elderly or a family member overwhelmed by the demands of caregiving. Signs that may signal an at-risk senior include isolation, depression, fear, poor hygiene, hunger and unexplained bruises or burns. Supplemental Nutrition Assistance Program: SNAP
Identifies and serves frail elderly persons, 60 years and older, who are at nutritional risk. They can receive a nutritious meal in their homes or at nutrition sites. Meals delivered five days a week and in some communities six or seven. Special dietary needs considered. 914.813.6300;

Telehealth Intervention Services
Trained technology students visit locations and take vital signs of seniors; data is then transmitted to telehealth nurses who review it remotely. If the tests show cause for immediate concern, the nurse will make contact. 914.813.6408

Veteran’s Service Agency
Westchester County Veterans Service Agency works with veterans, widows of veterans and parents of veterans to ensure they receive maximum benefits they may be eligible for; 914.995.2000;

Visions Services for the Blind and Visually Impaired
Vision rehabilitation and social service organization providing free training in the home and community for individuals of all ages who are blind or visually impaired. 888.245.3333;

WJCS Respite for Caregivers Escort Program
Enables caregivers to enlist an escort to accompany those 60 or older to a doctor visit, shopping trip, errand, movie or restaurant. Escorts do not drive. Funded by Westchester County Dept of Senior Programs and Services; service is free. Senior Crime Busters: 
Senior Crime Busters is a proactive elder fraud and crime prevention program that provides tips to seniors on how to stay safe and avoid financial exploitation and other scams. Seniors also learn how to protect their physical safety such as at shopping malls or walking in a neighborhood park. 914.995.2190

If you are concerned about your driving, or the abilities of a loved one, Burke’s Driver Evaluation Program can help. This assessment includes an in-clinic portion, where an occupational therapist administers tests that measure essential skills required to drive safely and, when appropriate, an in-vehicle test with a driving school follows. The goal is to measure safe driving ability and, when needed, recommend rehabilitation to strengthen skills. The program is offered at Burke’s Elmsford, Somers and Yonkers – Executive Plaza outpatient locations.

Assisted and Independent Living:

Coming off a very challenging 2020, especially within the senior housing communities, we are still seeing more senior living communities and retirement residences being built in Westchester and neighboring counties. There are more options – and they are not all the same. To help you maneuver through all the information out there, a first step is to understand the language used to help you differentiate between them when exploring the possibilities:
Continuing Care Retirement Communities (CCRC) accommodate the changing needs of residents. At a CCRC, someone may start off in independent living and then move into assisted living or nursing care if it becomes necessary. All the while, the individual stays on one main campus offering all the above. Independent Senior Living refers to apartment-dwelling in a senior community. Residents live independently and may be offered services such as meals and housekeeping. For those who need some help, there is Assisted Living. Meals, recreation, outings, and other programs are offered in addition to help with daily activities, nurses available 24/7 and wellness programs. Communities may even have indoor pools and other state-of-the-art equipment. Know that not all assisted living communities provide Memory Care services, while others specialize in this care. Respite Care provides caregivers a needed break and the time to focus on their own health and well-being: for themselves and for their loved ones. Hired caregivers can come into the home to provide respite for short or long periods of time. Alternatively, many assisted living residences provide respite care as well.  Nursing Homes & Rehabilitation:

For those not ready or able to go directly home when discharged from the hosptial – perhaps it’s a medication issue or inability to manage stairs, there is the option to go to a skilled nursing facility (SNF), where a doctor supervises care and there’s physical therapy along with other services. After a three-day hospital admission, stays at a SNF will usually be 100% covered for 20 days for those on Medicare.
