AGING LIFE CARE

According to the Aging Life Care™ Association, Aging Life Care Professionals, also known as geriatric care managers, bring a holistic, client-centered approach to caring for older adults or others facing ongoing health challenges. Working with families, the expertise of Aging Life Care Professionals provides the answers at a time of uncertainty. Their guidance leads families to the actions and decisions that ensure quality care and an optimal life for those they love, thus reducing worry, stress and time off from work for family caregivers through: assessment and monitoring; planning and problem-solving; education and advocacy; family caregiver coaching. AGING IN PLACE SERVICES

Aging in Place (also referred to as Living in Place) is the concept of remaining in your own home and community – safely and independently as you get older. Often times, in order to do this, assistance and/or other services are needed. Aging in Place Services are grassroots organizations, run by area residents, that help older adults stay in their homes. As the need for outpatient therapies continues to grow, Burke strives to meet the demand head on. Burke now operates nine outpatient clinics throughout Westchester and the Bronx and has plans to open an additional clinic in 2019. Senior living and retirement residences are continuing to crop up in Westchester and neighboring counties. But they are not all the same. Becoming familiar with the language used will help you differentiate between them when exploring the possibilities: Continuing Care Retirement Communities (CCRC) accommodate the changing needs of residents. At a CCRC, someone may start off in independent living and then move into assisted living or nursing care if it becomes necessary. All the while, the individual stays on one main campus offering all the above. Independent Senior Living refers to apartment-dwelling in a senior community. Residents live independently and may be offered services such as meals and housekeeping. For those who need some help, there is Assisted Living. Meals, recreation, outings, and other programs would be offered. There is help with daily activities, nurses available 24/7 and wellness programs. Communities may even have indoor pools and other state-of-the-art equipment. Keep in mind that not all assisted living communities provide Memory Care services, while others specialize in this care. Respite Care provides caregivers a needed break and the time to maintain their own health and well-being: for themselves and for their loved ones. Hired caregivers can come into the home to provide respite for short or long periods of time. Alternatively, many assisted living residences provide respite care as well. People are often not ready or able to go directly home when discharged from the hosptial. Perhaps it’s a medication issue or inability to manage stairs, resulting from hip or knee replacement surgery.
