AGING LIFE CARE

Aging Life Care Professionals® (as designated by the Aging Life Care Association) guide families towards the actions and decisions that ensure quality care and an optimal life for those they love, thus reducing worry, stress and time off from work for family caregivers. Services include: assessment and monitoring; planning and problem-solving; education and advocacy; as well as family caregiver coaching. Care managers may work independently, through certain care agencies, or with skilled nursing and rehabilitation centers and providers.

AGING IN PLACE SERVICES

Aging in Place Services are generally run by area residents, and help older adults remain in their homes: safely and independently as they get older. The types of services they provide may include: cultural and social activities; referrals to health care providers and tradespeople; transportation to shopping (please also refer to Transportation Services in this publication), personal care and medical appointments; health and wellness programs. Many of these Aging in Place services also welcome volunteers.
