AGING LIFE CARE

According to the Aging Life Care™ Association, Aging Life Care Professionals, also known as geriatric care managers, bring a holistic, client-centered approach to caring for older adults or others facing ongoing health challenges. Working with families, the expertise of Aging Life Care Professionals provides the answers at a time of uncertainty. Their guidance leads families to the actions and decisions that ensure quality care and an optimal life for those they love, thus reducing worry, stress and time off of work for family caregivers through: assessment and monitoring; planning and problem-solving; education and advocacy; family caregiver coaching. SAFE CENTERS FOR SENIORS Westchester County’s support for at-risk elderly or a family member overwhelmed by the demands of caregiving. SUPPLEMENTAL NUTRITION ASSISTANCE PROGRAM – SNAP Identifies and serves frail elderly persons, 60 years and older, who are at nutritional risk. They can receive a nutritious meal in their homes or at expanded congregate nutrition sites. Meals will be delivered five days a week and in some communities six or seven days a week. SUPPLEMENTAL SECURITY INCOME – SSI People with limited income aged 65 or older or who are blind or disabled at any age may be entitled to SSI in addition to Social Security benefits. This benefit provides monthly cash benefits to meet food clothing and shelter needs and automatically qualifies you for Medicaid. The amount of this benefit depends on a number of factors. WJCS ESCORT PROGRAM Enables caregivers to enlist an escort to accompany loved ones 60 years or older to a doctor visit, shopping trip, errand, movie or restaurant. Escorts do not drive. The Escort Program is funded by the Westchester County Dept of Senior Programs and Services and the escort service is free. There are so many options in Westchester for those who need or would like to leave their homes. Senior living and retirement communities are situated throughout the area, with varying levels of care and service. Many even allow short-term stays so you or a family member can try them on for fit. Here are some terms to be familiar with:

Continuing Care Retirement Communities (CCRC) accommodate residents’ changing needs. They may start off in independent living in single family homes/ apartments/ condos and can move into assisted living or nursing care if it becomes necessary. All the while, the individual stays on one main campus offering all the above.

Independent Senior Living refers to apartment-dwelling in a senior community. Residents live independently and be be offered services such as meals and housekeeping. For seniors who are independent but need some help, there is

Assisted Living. Meals, recreation, outings, and other programs would be offered in addition to meals. There is help with daily activities, nurses available 24/7 and wellness programs. There are assisted living communities that also provide

Memory Care services for their residents with dementia. Not all assisted living communities provide the specialized services associated with memory care.

Respite Care offers caregivers a much-needed break and the time to maintain their own health and well-being – for themselves and for their loved ones. Hired caregivers can come into the home to provide respite for short or long periods of time. Alternatively, many assisted living residences provide respite care as well.
